A Health Certificate Along with Other Documentation is Required for International Pet Transportation
All countries now require that animals coming from the United States have the proper documentation from your veterinarian and the USDA veterinarian before they will be permitted boarding on any airlines.
What is the Procedure for Acquiring a Health Certificate
Have your veterinarian complete the documents for the Country you are visiting, which if you are traveling to an EU Country is the new EU FORM 998. Most other countries the form that you need is the APHIS form 7001. If you do not have the forms for the country you are traveling to or need a form, contact World Pet Travel for the necessary documents.
Your vet will complete the form including vaccination information and that the pet is parasite free and in good health. The Veterinarian must then sign it and it must be signed and sealed by a Government official. In the US that is the USDA.
Send Documents Overnight
Take or mail the original of the form to the APHIS/USDA veterinarian office in your state and they will endorse and stamp them with the US seal. The fee is $24 per form...more than 1 animal can be listed on a form. If your vet uses separate forms for each animal, it will be $24 per form.
For countries where there is a titer or blood tests it is $76.00. These fees vary from country to country.
If there is not a State office located nearby you can send the forms by mail. Send them overnight with a return overnight envelope as this form is only good for 10 days. Include: The original health certificate that you receive from your veterinarian along with the rabies certificate along with payment in the amount of $24.00 or $76.00 per form (see above), a self-addressed, stamped overnight envelope for returning the form to you. Overnight is best for the quickest return. Each state has an APHIS/USDA state veterinarians office. ete this form.
